How to Host a Five Wishes Party

 

Leslie Piet, a registered nurse who works with cancer and other patients, offers these guidelines for hosting a Five Wishes Party:

Invite all adult members of your family and neighbors who are interested to come and learn how to complete their own Five Wishes. Consider asking guests to bring a covered dish to share in a good meal afterward.

For each guest, have at least one copy of the form you choose to use, and a few extra to send along to those who couldn't come.

Arrange for an expert or facilitator to attend to educate the group about the contents of Five Wishes, someone who can help bring up pertinent questions and elicit answers that family and friends need to know but are often afraid to ask. Large groups can be separated into smaller groups if more than one facilitator is available.

Once the Five Wishes are completed, signatures should be witnessed or notarized according to the laws of your state.

Copies of Five Wishes should be given to and discussed with the power of attorney, doctors, specialists and anyone else the individual wishes.  Keep the wallet card found in the back of Five Wishes indicating the location of the document.
